How Our Care Model Works Our model blends in-home care, telehealth, chronic care management, and long-term relationship-based support for seniors aging at home. Patients begin with an initial establishing visit conducted in the patient's home. During this visit, you would meet with the patient in person in their home while our provider participates via telehealth. Together, we establish the patient relationship, understand goals and risks, review needs, and begin developing an individualized plan of care. Following establishment, ongoing care is generally delivered through a combination of phone-based care management, telehealth, remote monitoring, and periodic in-home follow-up. You would help develop and manage Chronic Care Management (CCM) and Principal Care Management (PCM) plans of care, provide regular follow-up, coordinate care, and help patients stay connected, supported, and on track. Most ongoing interaction occurs remotely by phone and/or telehealth. However, we strongly believe in maintaining meaningful in-person connection with our patients. Most patients receive a monthly in-home visit. Some patients in more remote areas may receive quarterly in-home visits combined with more frequent remote support. This is not shift nursing or traditional bedside care. This is longitudinal, relationship-based nursing with significant autonomy, meaningful patient relationships, and the opportunity to proactively help seniors remain safer, healthier, and more independent at home. What You'll Do You'll work closely with seniors, families, providers, therapists, and caregivers to coordinate care and support successful aging in place. Responsibilities may include • Conducting initial in-home patient establishment visits alongside telehealth provider participation • Ongoing patient care management and relationship building • Phone, telehealth, remote monitoring, and periodic in-home patient follow-up • Developing and managing CCM and PCM plans of care • Care coordination with providers, specialists, therapists, caregivers, and families • Medication review and support • Chronic condition monitoring and patient education • Family communication and care plan follow-through • Documentation and care management activities • Identifying concerns, changes in condition, or emerging needs • Helping improve patient experience and care workflows as we grow This role combines clinical judgment, communication, problem solving, and relationship-driven care. 24/7 Access to Care Done Differently Our patients value knowing they have access to support when they need it.
