Responsibilities:
- Evaluate and treat patients in the home setting.
- Develop individualized plans of care.
- Provide therapeutic interventions and functional training.
- Educate patients and caregivers.
- Complete EMR documentation.
Requirements:
- Active NY license as a PT.
- Degree from an accredited PT program.
- Reliable transportation.
- Strong communication skills.
