📢 Now Hiring: Temporary RN Case Manager – Medicare D-SNP (Remote/California)
We're seeking an experienced Registered Nurse (RN) Case Manager to join a full-time temporary assignment supporting a Medicare Dual Eligible Special Needs Plan (D-SNP). This is an excellent opportunity for experienced case managers who are passionate about improving outcomes for complex Medicare populations.
📅 Assignment: July 2026 – December 2026 (estimated)
🕒 Schedule: Full-Time | 40 hours/week
📍 Location: California (RN License Required)
💼 Employment Type: Temporary (employed through a staffing agency)
What You'll Do
As an RN Case Manager, you will:
✅ Develop and manage individualized care plans for Medicare D-SNP members
✅ Coordinate care to improve clinical outcomes while promoting cost-effective services
✅ Educate members, families, providers, and community partners on the Case Management Program
✅ Collaborate with interdisciplinary teams to address medical, behavioral, and social needs
✅ Support quality improvement initiatives and program oversight activities
✅ Serve as a liaison between providers, community agencies, and the D-SNP program
Qualifications
✔ Active, unrestricted California RN license
✔ Associate Degree in Nursing (BSN may substitute for two years of required experience)
✔ Minimum 7 years of patient care experience
✔ At least 1 year of Case Management experience
✔ Minimum 1 year in an acute care or outpatient setting working with adult and/or dual-eligible populations
